Following is a completion of a student’s one-week fieldwork placement, we discussed her experience and my role as a supervisor. We created a quiz for family members and caregivers who are experiencing clients with stroke. I specifically asked about my clinical role as a supervisor to learn what I could change to best facilitate learning. I took this feedback and applied it to the next fieldwork student I worked with. I wanted to gain feedback at the end of future fieldwork experiences to continue to improve my ability to train, mentor, and educate Occupational Therapy students.
I’m working with another student who is in 3 months fieldwork placement in the University of Washington Occupational Therapy department to educate Occupational Therapy in an Acute Care Setting in September 2021. My plan is to set goals for a student to facilitate the learning process and educate a role as an Occupational Therapist. The student will finish the fieldwork in December 2021.
Below is the verification of fieldwork supervision and quiz forms from the student and I worked on together.

The quiz was designed by www.surveymonkey.com/
This quiz is designed for family members and caregivers with stroke survivors. This is beneficial to the discharge plan. The quiz is created to know if family members and caregivers and stroke survivors understand stroke to demonstrate safety at home. During treatments with supervision, one of the clients brought up where they want to know more about stroke and what to do before the discharge. The student and I then discussed the needs of the client, and we decided to make a simple quiz to assess how much the client know about each topic. This quiz was simple, and it was easily accessible to a client. In each room in the hospital, Bellevue WA, i-PAD was available. Before the discharge to home, we were able to provide this quiz and educate the client based on the results. This experience helped me to think about my competence project, what would be helpful to a client, families, and caregivers. It was an opportunity for me to think about clients’ needs and what would benefit them upon discharge from the acute care hospital.
