This artifact is a handout for clients with stroke, families, and caregivers for acute care settings upon discharge. It is intended to provide adaptive equipment recommendations and describe the key principles utilized.
This handout was created to help clients upon discharge to discuss adaptive equipment needs for safety awareness. I developed this handout with 3 different Occupational Therapists. We discussed twice a week, for approximately two months to decide which adaptive equipment is mostly needed upon discharge from the Acute Care setting Hospital. We then found pictures and added descriptions to approach easier ways to explain. After discussion with the Occupational Therapist team in the rehabilitation department, we decided to hand this handout to every client who is discharged from the hospital in Hayward, CA. This was an experience where I realized educating caregivers and families on safety awareness at home is an important role as an Occupational Therapist. This artifact helped me to develop an idea and concept of the Competence Project. Based on self-management theory, this artifact created a base of safety protocols in the Competence Project. During a workshop, this artifact was helpful to provide caregivers resources as well as provide knowledge on adaptive equipment for home safety.
